Kimberly Potter, APRN, FNP-c, FMCP-M, is a clinician with more than 19 years of clinical experience spanning acute care, cardiovascular medicine, ICU, and primary care. Over the years, she has become increasingly disheartened by the rising prevalence of chronic disease in young people and the rapid decline in function that she’s observed in older adults. She realized she no longer wanted to manage chronic conditions simply— she wanted to help prevent them.

As the founder of Proactive Functional Care, she now focuses on a systems-based, root-cause approach that prioritizes prevention and sustainable health solutions. Her mission is to empower clients to improve their well-being, extend their HealthSpan, and live more functional and satisfying lives—for themselves and their families.
